There are various expressions in Italian.

  • Capo dei capi = boss of the bosses
  • Capo di tutti capi = boss of all bosses
  • Capo de tutti i capi = boss of all the bosses

The reference may indeed be to a very powerful crime boss, but this is not someone with a specific role in any particular organized crime group.

Rather, the "boss of all bosses" is any murderer or robber or extortioner essentially in the very act or commission of the crime, without regard to any organized mafia hierarchy. And this is what makes that person "the boss," that the victim is either being killed or is desperate and willing to do anything to save his or her own life.

Gen. 4:6–7. And the Lord said unto Cain, Why art thou wroth? and why is thy countenance fallen? If thou doest well, shalt thou not be accepted? and if thou doest not well, sin lieth at the door. And unto thee shall be his desire, and thou shalt rule over him.

The Lord warns Cain that he will have his way with his brother and kill him if he persists in his anger — to "rule over him" like a mob boss or a made man in the mafia.

And in a larger sense, the "boss of all bosses" is precisely that survivalism or desperation to live when confronted with mortal danger of any sort, or faced with any life-or-death situation, criminal or not.

If there's a godfather — assuming these people are baptized — is not God the Father Himself the boss of all bosses? But that's the movie, from the liberal Jewish Hollywood industry which requires circumcision and despises baptism, and therefore portrays those as evil who would be present at a baptism: and not merely Roman Catholics, as might seem typical of Italians, but those of any other Christian denomination or calling as well.
