Does a woman really have “cancer” or does she have a Best Friend Forever ® who would prefer for the purposes of such friendship that she have her breasts cut off?
The crime of mayhem is not based on "permission" or "consent" or the lack thereof.


That's a lot of doctors cutting off breasts.
Perhaps it is considered risqué or indecent in some situations to expose oneself too much with skimpy clothing. I cannot really consider nudity to be a criminal matter; the natural state in which we are born and die cannot violate any natural laws, so I would leave the moral issues and the preaching of them up to the religious ministers, and keep the law out of it, because literally cutting off body parts which are somehow deemed offensive on religious grounds is not really the right medical answer in the context of cancer as a moral diagnosis of risk or vice.
The radical mastectomies among other unnecessary surgeries are a symptom of a broader social problem where certain women who hug and kiss each other too much are all but going to law to cut each other's breasts off, because that sort of affection is unnatural.
The natural law does not generally permit body parts to be cut off without an immediate life-saving justification.
Many women would feel more natural and secure hugging and kissing a bigger and stronger person who does not have breasts to compete with her own — namely, such a person would typically be male. Certainly there are men (many if not most, in fact) who would permit their hair to be cut off by such women, but probably not healthy living body parts.
